Menopause, and what it does to skin

Hormones shape skin all through life. But no change is larger, or less understood, than the one that comes with menopause. And most of what you read stops at a single hormone. The real picture is more interesting than that. Estrogen, progesterone and androgens all decline together, and what changes skin is the balance between them.

Menopause and skin: it is not only estrogen

Estrogen gets the attention, and it matters. But at menopause, estrogen, progesterone, and androgens all decline together. What really changes is the balance between them.

Estrogen falls the fastest and the furthest. At the same time, a protein called sex hormone-binding globulin, which normally keeps androgens in check, drops as well, leaving those androgens freer to act. So even though androgen levels themselves are falling, their relative influence rises.

This is why menopause can bring two changes that seem to contradict, at the very same time. Skin that is drier and less firm, and skin that suddenly breaks out.

The dryness, the thinning, the loss of firmness, these come from the estrogen side. The breakouts along the jaw, or a coarser facial hair, come from the relative androgen side. One shift in balance, two very different faces.

What the loss of estrogen does to skin

Estrogen does a great deal for skin. It supports the collagen that keeps skin firm. It raises the molecules that hold water in the dermis, including hyaluronic acid. It supports elasticity and the barrier, and it even behaves as an antioxidant.

So its decline is felt widely. In the first five years after menopause, skin loses around 30% of its collagen, and the loss tracks the years since menopause more closely than age itself. Skin holds less water, feels thinner, what many women describe as crepey, and recovers more slowly.

This is where the difference between hydration and moisturization stops being a technicality. Skin that holds less water needs both: something that brings water in, and something that keeps it there.

Why menopause can bring breakouts, not only dryness

On the other side of the balance, the relative rise in androgen influence shows up in the oil glands and the hair. Sebum can increase in places, which is why some women see breakouts return in their forties and fifties, often along the jaw and chin. Coarser facial hair can appear for the same reason. It is common, it is hormonal, and it is not a failing of hygiene or of care.

Perimenopause: where the change begins
